Completing Your Sign-On Package - as easy as 1-2-3-4!

The following is the entire sign-on package to be completed in full and returned to the Human Resources Department no later than the date specified in the email sent to you along with access to this site. Follow the 4 steps below to complete your sign-on package.

 

PLEASE NOTE: failure to complete this entire process will result in a delay in your pay and benefits enrollment.


  Step 1 - New Employee Checklist

Download and print this checklist to ensure your sign-on package is completed successfully.


  Step 2 - Provide Photocopies

Please make photocopies of the following personal documents:

    1. Social Insurance Card - In the absence of a SIN Card we will only accept a "Notice of Assessment" tax form from Revenue Canada.
    2. Driver's License
    3. Birth Certificate - In the absence of a Birth Certificate we will only accept a passport or citizenship card.


  Step 3 - Complete the Following Forms

Please print off and complete the following forms:

    1. Employee Information Form
    2. Direct Deposit Authorization Form 
    3. 2010 Federal TD-1 Form
    4. 2010 British Columbia TD-1 Form
    5. British Columbia Health Care Form
    6. Life Insurance Beneficiary Form
    7. Health Insurance BC - application for group enrollment 
    8. EPCOR Standard Policies - only return the pages that require a signature.
    9. Pension Plan - please refer to your offer letter to confirm which Pension Plan you will be enrolled in.
      -- LAPP (Local Authorities Pension Plan) - You will receive further information directly from LAPP after you commence employment.
      -- DCPP (Defined Contribution Pension Plan)Please complete the Defined Contribution Pension Plan Enrolment Form

 
For Port Hardy Employees only:

Please print off and complete the following forms if you are a Port Hardy EPCOR employee:

    1. Municipal Pension Plan - Nomination of Beneficiary
    2. Municipal Pension Plan - Pension Enrollment Election


  Step 4 - Complete Sign-On Process

Please assemble all documents and return to EPCOR Human Resources via email (careers@epcor.ca), fax (780-412-7602), or mail to:

Driver's license photocopies are best sent via email, as faxes do not normally come through clearly.

 

All forms must be returned no later than the deadline specified in your offer of employment email.
